Marian Manor is a long term care facility that provides care on a twenty-four hour basis to those needing either skilled, intermediate, Alzheimer's or personal care. The Interdisciplinary Healthcare Team is an integral part of the nursing care provided for the residents in the nursing facility.  The team consists of representatives from nursing, social services, activities, dietary, therapy and pastoral care.  The Interdisciplinary Healthcare Team performs a comprehensive assessment on admission and then quarterly and annually to develop a plan of care based on the resident's needs.  We encourage the attendance and participation of the resident and her/his family at the care plan meetings.  Our goal is to keep each resident at her/his highest level of functioning and to promote as much as possible the resident's independence.
